Clinics overview
Your accountant
separates VAT-exempt
and taxable services
A private clinic mixes patient appointments, payments at reception, invoices to mutuas and insurers, clinical supplies and often professionals who send invoices to the company. The VAT isn’t the same across the board either: diagnosis and treatment are usually exempt, purely cosmetic work isn’t.
You photograph receipts or upload invoices and mutua settlements to the Numonis platform, and your card terminal reports arrive on their own. Our software suggests the amount, the VAT or exemption and the date. Your accountant confirms them and keeps your VAT, tax and Social Security filings on time.
What we handle
The paperwork after the patient leaves
Your accountant handles the tax and accounting, and a labour specialist the payroll, for dental, physiotherapy, psychology and aesthetic medicine clinics.
-
Exempt healthcare
Diagnosis, prevention and treatment by health professionals are usually exempt from VAT, whoever pays: the patient, a mutua or an insurer.
-
Aesthetics at 21%
Purely cosmetic treatments and products sold at reception usually carry 21%. Your accountant keeps them separate on your invoices and in your Modelo 303.
-
Pro rata VAT
If you mix exempt and taxed work, your accountant deducts the VAT on supplies, rent, software and shared equipment in proportion, under the prorrata.
-
Patients, mutuas and insurers
We make sure every treatment has an invoice, with the exemption stated where it applies, and match each payer’s settlements with the bank.
-
Clinic staff
Your labour specialist prepares contracts and payslips for hygienists, assistants, reception and nursing under the provincial or regional agreement for your area.
-
Withholding and taxes on time
Withholding on your professional fees and on the collaborators you pay, the 111 and 190, and your Renta or corporate tax.

FAQ
What people ask before joining Numonis
Does aesthetic medicine always carry VAT?
Treatments with a purely cosmetic aim, outside the diagnosis, prevention or treatment of an illness, are taxed at 21%. If a treatment corrects an illness, injury or congenital defect it can be exempt; the tax authorities look at each case.
Do I file the Modelo 303 if all my work is exempt?
If you only carry out exempt work with no right to deduct and nothing taxed, the 303 may play little part in your VAT calendar. Once you add aesthetics, product sales or any other taxed work, you file it, with the prorrata where it applies.
What withholding applies to a self-employed dentist or doctor?
Generally 15% on professional income. The 7% rate for new professionals can apply in the year you start and the two following years, if you meet the requirements and tell the payer.
Does Numonis connect to my clinic software or card terminal?
Yes. Your clinic management software, your card terminal and Stripe connect to the Numonis platform, and their reports and settlements reach your accountant on their own. Tell us which software you use and we’ll connect it.
How much does it cost for a small clinic?
It depends on whether you work as an autónomo or through a company, how many transactions you move and how many people you employ. Plans for autónomos start at €49 + VAT a month, plans for companies at €99 + VAT, and payroll at €45 + VAT a month for the first person.
